The Medical Center Community Diabetes Program
The Medical Center Community Diabetes Program offers the highest quality diabetes education in the area to assist you and your family. Diabetes education will help you to find the balance you need to live a healthy life and prevent the complications that may occur from diabetes. The Medical Center Community Diabetes Program follows the high standards for education recommended by the American Diabetes Association. The program is staffed by healthcare professionals with specialized training and commitment to diabetes care. A Certified Diabetes Nurse Educator and a Registered Dietitian are on our staff to provide you with personalized service. Our program is designed to work closely with your personal physician and his or her treatment plan. Education Opportunities The Medical Center Community Diabetes Program offers flexible education opportunities to meet your busy schedule. A dynamic comprehensive class setting is offered during day and evening hours to meet your specific needs. Our program provides assistance and instruction on:
Individual counseling with a Certified Diabetes Nurse Educator or Registered Dietitian is available by appointment for anyone with special needs. Entering Our Program You may enter the Community Diabetes Program through a referral from your physician or by self-referral. Your physician will then be contacted by our staff to assist in the development of your personal plan of care. Your physician is an integral part of your diabetes care and will be kept informed of your progress throughout the program. In addition, we encourage you to bring a family member or friend with you to our program as additional support. The Medical Center is pleased to offer the Community Diabetes Program at an affordable cost. Some insurance companies will cover the cost of the program. Please check with your insurance provider to see if you qualify. Staying With You Once your initial program has been completed, your road to success in diabetes care continues. Although the formal program is finished, we will still be available to assist you in a variety of ways. You may expect the following:
