While the pre-registration process cuts down on the registration time, there is still some important information you will need to bring with you the day of your procedure. Please bring the following:

• All health insurance cards, certifications and claim forms
• A form of identification (such as a driver’s license)
• Any physician’s orders and test results given to you by your physician’s office
